SPLASH COVID 19 DECLARATION OF HEALTH,
PRIOR TO STARTING LESSONS
I declare:
-neither I, nor my child is displaying any symptoms associated with Covid-19.
-neither I, nor my child has come into contact with any confirmed cases of Covid-19.
-I understand that children can no longer avail of missed lessons and no refund is available for lessons missed.
Please note if a confirmed case of COVID-19 is confirmed in your child’s school, you should apply the same level of caution as required by the Department of Education. If the school permits your child to attend daily classes, we are happy to permit your child to swim. If your child is asked to stay out of school, due to close contact with a confirmed case of COVID-19, we request that you do not attend lessons.
I declare:
-it is my responsibility to keep my child off lessons, if they are displaying any symptoms of Covid-19 as per associated HSE material.
-it is my responsibility to inform SPLASH swim school that we will not be attending lesson
if they are displaying any symptoms of Covid-19 as per associated HSE material.
-I will reply BY EMAIL to info@splashswim.ie if we cannot attend lessons.
